Vital Statistics

The Borough's Office of Vital Statistics is open Monday through Friday from 8:30 a.m. to 3:30 p.m.
The Office of Vital Statistics is responsible for the issuance of Certified Copies of Births, Marriages, Remarriages, Civil Unions, reaffirmations of Civil Unions and Domestic Partnerships; as well as applications for licenses for Marriages, Remarriages, Civil Unions, reaffirmations of Civil Unions and Domestic Partnerships for the residents of Farmingdale or for those hosting their events in Farmingdale.

** IMPORTANT: Please note - Events that occur at Eagle Oaks Country Club must be recorded as occurring in and then be filed with Howell Township. Events that occur at Allaire Chapel must be recorded as occurring in and then be filed with Wall Township. The Borough of Farmingdale cannot accept these records** 

APPLICATION FOR LICENSE: Marriage, Remarriage, Civil Union or Reaffirmation of Civil Union

All licenses are available to all qualifying couples of all gender compositions, including applicants seeking marriage licenses for same-gender or "same-sex" marriages and nonbinary applicants. 

Couples holding their event in the State of New Jersey must apply for their license at least 72 hours before the intended date of the event.
Application is made in the municipality (town) where either applicant resides. If you are a resident applying in Farmingdale, please bring the following required documents to your appointment when applying for a license:

  • Proof of identity by presenting your driver’s license, passport or state/federal I.D.
  • Proof of residency (in Farmingdale) by one applicant if a New Jersey resident
  • Social Security number*
  • A witness, 18 years of age or older
  • The $28 application fee (cash, check or money order)
*Social security number is required by law for US citizens and will be kept confidential.

Any documents in a foreign language must be accompanied by a certified English translation.

You may prefill the first two pages of the application prior to your appointment, the third page must be completed in the presence of the registrar.
The license is valid for 30 days from the date of issuance (when the license is picked up).

CERTIFIED COPIES
The Vital Statistics Office maintains and records all births, marriages, remarriages, civil unions, reaffirmations of Civil Unions, and deaths that occur in Farmingdale only
Confidentiality - All vital statistics are extremely confidential and anyone requesting a document must adhere to State requirements to obtain the requested records by providing proper identification and proof of relationship.  
To obtain a Certified Copy you must complete an application for a certified copy and provide proof of relationship to the record.
Certified Copies are $15.00 each (cash, check, or money order).
